Are you passionate about helping others? We are looking for a compassionate and dedicated Remote Bilingual Care Coordinator (Certified Medical Assistant) to join a dynamic team. As a Care Coordinator, you will play a key role in improving the lives of patients by providing high-quality care coordination services. You will work closely with patients, their families, and healthcare providers to ensure patients receive the best care possible, especially those with chronic and behavioral health conditions.

Key Responsibilities:
Provide monthly care coordination services to an assigned panel of patients, including conducting phone calls to ensure that patients' needs are met between office visits.
Collaborate with healthcare providers, pharmacies, and community programs to develop personalized care plans for patients.
Assist patients in managing their chronic conditions by educating them on their health and coordinating treatment plans.
Handle a high volume of phone calls (30-50 calls per day), aiming to achieve 15 patient interactions to complete 20 minutes of care coordination services each month.
Identify social determinants of health, gaps in care, and patient eligibility for additional services, while collaborating with relevant community programs and resources.
Ensure patient privacy and confidentiality, while adhering to legal and regulatory standards.
Who You Are:
You are a clinical professional with at least 1 year of experience as a Certified/Registered Medical Assistant.
You are fluent in both Spanish and English, with strong verbal and written communication skills in both languages.
You are tech-savvy, with experience using multiple EMR/EHR systems and familiar with Microsoft Office and Google Suite.
You possess exceptional organizational skills, are self-motivated, and work independently with a high level of professionalism.
You have the ability to manage multiple priorities and meet productivity goals while providing high-quality care to patients.

Requirements:
At least 1 year of experience as a Certified/Registered Medical Assistant.
Fluency in Spanish and English, with excellent verbal and written communication skills.
Proficiency with EMR/EHR systems, Microsoft Office, Google Suite, and a quick learner of new software.
Strong phone demeanor, with the ability to engage patients compassionately and professionally.
Ability to work independently in a remote environment and stay focused on patient needs.
Experience in a call center or triage setting is a plus.
Case management or care coordination experience is a plus.
Experience working with a geriatric population is a plus.
General Working Conditions:
You will work a full-time 40-hour schedule, Monday through Friday.
You must have a private home office that allows you to maintain confidentiality and focus on patient care.
The role requires a second monitor, reliable high-speed internet (50 Mbps or higher), and a quiet, distraction-free environment.
